CCRI commissioned to examine integrated care models for older people living on the Central Coast

In partnership with international consultancy Rebbeck the CCRI has been commissioned to provide expert support to Hunter New England and Central Coast Primary Health Network to provide guidance on the development of integrated care models that meet the priority needs of older people living on the Central Coast. This includes key strategies related to end-of-life care, dementia, chronic disease management, cancer care pathways and improving the effectiveness of care transitions for older people to/from hospital settings to community-based and home settings.
