Expert consultancy
A key priority for the CCRI is to enable the translation of evidence into policy and practice. The CCRI supports this ambition through the provision of expert support and consultancy services that can offer end-to-end methodology in the design, implementation and evaluation of integrated care projects and programs.
The CCRI’s approach is based on the evidence-informed Implementation Model for Integrated Care that was developed through research projects undertaken with the European Commission and World Health organisation Europe.

Diagnostic services
Plans for the introduction of new models of integrated care to specific individuals or cohorts of people first requires a population health needs assessment to be developed that also includes a situational analysis of the resources and competencies available and needed to address such needs. Based upon a published and validated framework to assess system competencies in delivering integrated care, the CCRI has developed an Integrated Care Initiative Tool to support an understanding of current capabilities and inform future action. The tool was recently used to evaluate the success of the Central Coast’s integrated care program.
Design & Implementation support
The CCRI is able to assist clients in the design of their integrated care programs through supporting the development of a costed value case for their initiatives and a process to enable the development of strategic plans. Often evolving from the results of the diagnostic service, the CCRI’s design services seek to bring together the best available international evidence together with the needs and capabilities of the local context. This can be done intensively in a 3-5 day process with a core team, or can operate on a longer time period to support a process of co-design.
Recent clients include the Hunter New England and Central Coast PHN where the CCRI is working with Rebbeck to support their thinking on future models of aged care.
Evaluation services
As experts in the research and evaluation of integrated care projects and programs, the CCRI can support clients in developing monitoring and outcome frameworks through which to judge the impact and effectiveness of integrated care innovations over time. The CCRI can also draw down from the expertise of its academics at the University of Newcastle and other partners to bring together research teams to directly evaluate integrated care initiatives. The CCRI is particularly keen to ensure such evaluations focus on the value of integrated care.
