SUMMARY

To support Hunter New England and the Central Coast PHN develop its strategy to address improvements in palliative and aged care, this project conducted an evidence review to understand how integrated care models can support health and aged care for older people living at home. The review considered the evidence for effective implementation of models of care across a range of key needs including: Aboriginal and Torres Strait Islander health; intermediate care; chronic disease management; dementia care; end of life care; tackling social isolation; suicide and mental health; cancer screening / pathways; and out-of-hours care.
