Centre of Research Excellence on Integrated Health and Social Care

The purpose of this Centre of Research Excellence (CRE) is to improve the health of vulnerable people in the community, and to advance the translation of health, economic and social systems research into policy and practice. A particular focus will be on reducing hospitalisation through innovative, high quality, collaborative research of home and community-based service systems, including the development of digital and virtual modes of community-based service delivery. The research will build on our previously acclaimed work within the Sydney Local Health District (SLHD) – the Health Homes and Neighbourhoods Integrated Care initiative. In partnership with the International Foundation for Integrated Care, the CCRI shall be supporting the CRE in knowledge sharing and education and training opportunities arising from the CRE’s work.
The CCRI supports CREHSCI in providing leadership, project management and the secretariat to the International Foundation for Integrated Care (IFIC) Australia initiative designed to create an independent and inclusive network that inspires, influences and facilitates the adoption of integrated care in policy and practice.
